Application Process

The following is the required process for application to Colegio Roosevelt.

Step 1

Complete the Application Online Form

After completing the Application Online Form you will receive a confirmation email. In the email you will find a link that will take you directly to the section to start uploading the required documents.

Step 2

The following academic records and required documents must be uploaded and submitted in step 2 in the Application Online.
 

REQUIRED DOCUMENTS FOR ALL APPLICANTS
International Applicants: Copy of the Passport or Residence Card and Birth Certificate
Peruvian Applicants: Copy of Birth Certificate and current "Documento Nacional de Identidad (DNI)"
Vaccination Records including Hepatitis B  
FDR Confidential Family Reference
Parents Employment Presentation Letter
Copy of Both Parents Personal Identification Document (DNI, Residence Card or Passport)
Application and Advanced Entrance Fee Deposit

EARLY CHILDHOOD, KINDERGARTEN AND GRADE 1 APPLICANTS
Applying to EC3, EC4, Kinder and Grade 1 (first semester)
Report Cards from Nursery School (if applicable)
Parent Questionnaire
Applying to Grade 1 (second semester)
Report Card of First Semester of Grade 1 in their original language accompanied by its respective translation into English if necessary
Report Cards from Nursery School and Kindergarten (if applicable)
School Counselor's or Principal's Recommendation Form
Homeroom Teacher´s Recommendation Form

GRADE 2 - GRADE 5 APPLICANTS

Standardized Test Results ( Measures of Academic Progress - MAP)
Homeroom Teacher's Recommendation Form
School Counselor's or Principal's Recommendation Form
Documentation of Other Educational Services (e.g. IEP or Formal Educational Plan, Psych-ed Report, Counseling Report, Physical Therapy, Learning Support (LS), English as an Additional Language (EAL), Gifted and Talented programs, etc.)

GRADE 6 - GRADE 12 APPLICANTS

Grade 6 to 8 - Last three (3) years Report Cards (including final grades) in their original language accompanied by the respective translation into English if necessary
Grade 9 to 12 - All Transcripts and Report Cards (including final grades) from grade 6 and up in their original language accompanied by the respective translation into English if necessary
Standardized test results (e.g. Iowa, Otis-Lennon, SSAT, PSAT, Measures of Academic Progress - MAP)
English Teacher's Recommendation Form
Math Teacher's Recommendation Form
Middle School Counselor's or Principal's Recommendation Form
High School Counselor's or Principal's Recommendation Form

Step 3 - Other Evaluations

The Office of Admissions will schedule applicants evaluations or placements interviews with our school principals/counselors/learning support team (if necessary).

Once the required documents are complete, the Office of Admissions will help you set up a parents’ interview with the Director of Admissions.

Step 4 - Admissions Decision

Parents will receive an email notification regarding the admissions decision.
