Overseas Faculty Manual - Documents Needed to Obtain Work Permit and Resident Visa

IMPORTANT :  ALL OF YOUR DOCUMENTS  MUST HAVE THE
SAME NAME AND SIGNATURE AS IN YOUR PASSPORT

You will be coming to Lima on a Tourist Visa

The steps towards obtaining the approval of your Contract of Employment and Resident Visa are:

• Complete and send Data Sheet to Human Resources
• Passport (valid through August 2012, must have at least three consecutive blank pages).
• BA/MA Original Diploma authenticated/legalized by the Peruvian Consulate.
• Teaching Credential (copy).
• Original Marriage Certificate (if spouse is your dependent or if your diploma is written under your maiden name) authenticated by the Peruvian Consulate
• Original Children’s Birth Certificates authenticated by the Peruvian Consulate
• Children’s Adoption Decree of Letter of Guardianship (if applicable) authenticated by the Peruvian Consulate
