How to Apply

The following is the required process for application and enrollment at Franklin D. Roosevelt School. Application for enrollment is NOT OFFICIAL until all items requested have been provided.

  1. Contact Admissions Officer, Ms. Nora Marquez (nmarquez@amersol.edu.pe)
  2. Complete Online Application (click here)
  3. Complete and return Permission to Release Records form (click here)
  4. Contact your child’s current school. Records must be sent by the previous school (translated into English if necessary) in a sealed envelope including:

    • An official copy of the last three years of records
    • A school profile
    • The Recommendation Form to be completed by school personnel: Elementary (click here) Middle School/High School (click here)
    • Standardized test results (e.g. Iowa, Otis Lennon, SSAT, PSAT)
    • Report of special needs programs (IEPs, ESL, Gifted Programs, etc.)

  5. Complete Financial agreement form (click here)
  6. Parent’s Agreement to Participate form (click here)
  7. Once the above steps are completed and you have arrived in Lima, please:

    • Schedule an appointment to visit the school, submit forms and review paperwork. Through the Office of Admissions, schedule an appointment for your child’s interview
    • In addition to the US diploma, the candidate may choose enrollment in the Official Peruvian Program (click here)
    • Pay application and entrance fee deposit (click here)
    • Submit:

    - Two recent color photographs of the candidate (3x2.5 inches)
    - Copy of candidate’s passport or birth certificate- Medical certificate from your child’s doctor and copy of complete vaccination records (including hepatitis B)
    - Parent’s employment verification

  8. Parents will be notified about the decision by the Admissions Officer.
