Copy of the candidate’s passport or birth certificate
Medical certificate from your child’s doctor and copy of complete vaccination record (including hepatitis B)
Records sent by previous school (translated into English if necessary) or hand-carried in sealed envelope including:
Past three years (where applicable) and most current transcripts/report cards/mark sheets
Standardized test results if taken (e.g. IOWA, OTIS LENNON, SSAT, PSAT)
Report of special learning needs or programs (IEPs, ESL, gifted, etc.)
If the candidate plans to enroll in the Official Peruvian Program – OPP - (depending on his/her needs or interest):
Students coming from local schools:
- Official Peruvian Certificate (original)
- “Ficha Unica del Estudiante”
Students coming from abroad:
- Official Certificate validated by the School District/Ministry of Education of country of origin and by the Peruvian Consulate of that country. If necessary, the Certificates must be translated into Spanish
Applicants for grade 1 and up also need to submit letters of recommendation from Math and English teachers and
Principal or Counselor. Where appropriate, recommendations from English and Math teachers may be replaced by
one recommendation from the Homeroom Teacher.
